Bring the following with you: one (1) form of identification (government-issued photo ID), which can include your license, passport or ID and one (1) completed form from each approved BOE, charter or private school. You will also need to bring your Regional Educational Service Center (RESC) Fingerprint Permission Form and the applicant tracking number from the CCHRS Pre-Enrollment System.
