RTS Referral Process
The Referral Packet is sent by the sending district to EASTCONN's Director of Pupil Services. Referrals need to be received by May 1st of the current school year.
Referrals should contain the following:
- Two previous Individual Education Plans (IEPs)
- Recent evaluations, including transition assessments
- Parent contact information
- Review of Records to be completed by EASTCONN staff
Criteria for RTS:
- Students ages 18-22 with a disability who require transition services beyond high school
- Students have met all academic requirements
Student & Family Visit to RTS:
- EASTCONN coordinates a time/date for a visit to our program
- If appropriate, EASTCONN coordinates a Director's visit to the student's sending school
- Student completes a shadow day at RTS
- EASTCONN staff makes a determination about acceptance into RTS and notifies the sending district
- Transition PPT to occur with EASTCONN participation Intake Packet needs to be completed and returned prior to student starting at RTS
